1. A valid passport (with a Seven-Nine month validity on the date of the application) & all previous Passports.
2. Recent Photo 2 Copies (35”x45”, White background, Matt/Glossy Lab Print).
3. National ID card Photocopy. (Both Side)
4. Birth Certificate Photocopy (Only for Child & Infant).
5. Income Tax Certificate & TIN Certificate.
6. Letter of Invitation (If Have but not Mandatory)
7. Inviter Details (Ex: Relationship between applicant, Contact Details, Address, Passport Copy, Permanent Resident Card, Bank Statement, Any Utility Bill Copy).
8. V Mention (Present Address, Contact Number, Email address, Last Educational qualification, Name of previous organization.)
9. No Objection Certificate (Job Holder)
10. Renewal Trade License. (Business Person)
11. O (Government Order) for Official Passport
12. Joint Venture Company or limited company’s Partnership deed or Memorandum.
13. BMDC certificate. (Doctor)
14. BAR Council Certificate. (Advocate)
15. Retirement Certificate (Government Employee)
16. Company Letter Head Pad (Business Person)
17. Visiting Card (Business Person & Job Holder)
18. Student ID card Photo Copy
19. Bank Statement for the Last Six (06) Months (Must have Bank Seal & Signature)
20. Bank Solvency Certificate (Must have Bank Seal & Signature)
21. Date and reason for previous refusal. (If any)
22. Marriage Certificate Copy (If Spouse name not mentioned in Passport)
23. Travel Insurance.
